Changing your GP
If you want to change your doctor you can do this at any time, you do not have to have a special reason. Just complete your medical card and give it to your new GP practice. If you do not have a medical card the new surgery will provide you with a substitute form, which the doctor will sign if willing to accept you as a patient. You may go to your new practice for treatment as soon as the doctor has accepted you.
If you are unable to secure acceptance after applying to at least three different practices, you may contact our Patient Data Department on 0151 296 7000 to request an assignment application form. On this form you will be required to provide a list of GP Practices that have refused to accept you on the list, plus full details of each person requiring assignment, i.e. names in full, maiden name (where applicable), date and place of birth, previous address and name of previous doctor.
Unfortunately, it is not possible to comply with any requests for assignment to a specified practice.
If you are not registered on the list of a local GP Practice because, for example, you have recently moved into the area and require treatment urgently but have been refused acceptance, you can apply to any doctor to provide “immediately necessary treatment” for a period of 14 days, or less, in the event that you are accepted by a doctor or allocated to one within that time.
Finally, to avoid difficulty in the event of treatment being urgently required, you should register with a Doctor’s practice without delay. Do not wait until you become ill before doing so.
